Once automated with the website I can revise the order form.  The clinician will need the information from pages 2 and 3 for pricing of the antigens. There are 2 separate memberships they can order from.  If there is a gateway to allow them to place an order once they purchase the internal microbe and/or allergen membership for the specific antigens listed under that memberhips?
